What a Biological Age Test Can and Can’t Tell You
A biological age test estimates how old your body looks on the inside, using markers in your blood or DNA, and compares that estimate with your calendar age. Used well, it is a useful snapshot for setting priorities and tracking direction over months, but it cannot diagnose a condition, predict your lifespan or replace a doctor’s review.
Biological age has become a common talking point in Dubai, from clinics to social feeds. Here is what the number measures, where it falls short, and how to use it sensibly alongside your training.
What a biological age test measures
Chronological age is the number of birthdays you have had. Biological age is an estimate of how your body’s systems are functioning compared with the average person of the same age. Most tests fall into two broad families.
Blood-chemistry clocks. These combine routine markers, such as blood sugar, kidney and liver markers and inflammation, into a single score. Levine and colleagues (2018) first built a “phenotypic age” from routine blood chemistry, then an epigenetic version that predicted all-cause mortality, healthspan and physical functioning better than earlier clocks.
Epigenetic clocks. These read DNA methylation, chemical tags on your DNA that shift with age, usually from a blood or saliva sample.
Some services also blend in fitness or body composition measures. Each provider uses its own formula, so a score from one test is not directly comparable with a score from another.
What the result can tell you
Whether your markers look older or younger than expected for your age, at that moment.
Which systems may be pulling the score up, when the report breaks it down, for example blood sugar control or inflammation.
A baseline to compare against when you retest with the same test, under similar conditions, months later.
A clear reason to take sleep, training and nutrition seriously, which is often its most practical value.
What it cannot tell you
A biological age score is a statistical estimate built from population data. It describes risk at group level. It cannot tell you when or how you will age, and a younger result is no guarantee of good health.
Precision is the other limit. Higgins-Chen and colleagues (2022) found that technical noise alone produced differences of up to 9 years between repeat measurements of the same sample across six well-known epigenetic clocks. Their improved method brought most repeats within 1.5 years, but not every commercial test uses it. A swing of a few years between two results can be measurement noise rather than real change.
It also cannot diagnose anything. If a report flags something that worries you, take it to a qualified healthcare professional, who can interpret it alongside your history and order the right tests.
Can training change your biological age?
The evidence is promising but modest. In the DO-HEALTH trial, Bischoff-Ferrari and colleagues (2025) followed 777 older adults for three years, testing vitamin D, omega-3 and a simple home exercise programme. Omega-3 slowed several clocks, and the three together had a small additive effect on one of them, PhenoAge. The effects amounted to roughly three to four months over three years.
Two lessons follow. Real change is slow and small, so be sceptical of claims that a few weeks of anything took years off. And the habits that matter for biological age are the same ones that matter for everything else: regular strength and cardiovascular training, consistent sleep, enough protein and fewer very late nights. Speak to your doctor before starting any supplement.
How to use a biological age test well
Test at a calm moment. Avoid the days after a long-haul flight, an illness or a heavy weekend, and follow the preparation instructions you are given.
Retest with the same method. Compare like with like, every 6 to 12 months, since meaningful change takes time.
Read the components. The individual markers are where the useful actions sit, more than the headline number.
Track what moves weekly. Strength, measurements, sleep and daily steps show progress long before a retest. See what to track beyond the scale.
Keep it in perspective. Treat the number as a direction to work on, and leave the verdicts to your doctor.
How EVOLVE approaches this
Every EVOLVE programme starts with an Audit that includes a blood panel covering 61 biomarkers and a Biological Age Test. Results are reviewed with you as coaching information. Coaches do not diagnose, and anything clinical is referred to a qualified healthcare professional. You can read more about why blood testing helps a coach plan.
From there the method runs in order: a Plan built around your goals, schedule and starting data; Execution in 45-minute 1:1 sessions; Tracking of strength, body composition and habits; and Adjustments at regular checkpoints. The biological age result becomes one reference point among several, retested later to confirm the direction.
Frequently asked questions
Is a biological age test accurate?
It is a reasonable estimate. Results can differ by a few years between tests or providers, so compare results from the same method and focus on the trend over time.
How often should I retest?
Every 6 to 12 months is enough for most people. Meaningful change is slow, and testing more often mostly measures noise.
Can a biological age test replace a medical check-up?
No. It complements a check-up. A doctor’s review of your history, examination and standard tests remains the reference for your health.
Does strength training help as you age?
Regular strength training supports muscle, strength and everyday function as you get older, and it is one of the habits most worth keeping. Here is how to build a strength routine after 40.
